Completing your paperwork before you arrive at one of our New Jersey pain management clinics will help to speed up your check-in process. Please print out our new patient form and bring the completed form with you to your first visit. If you would rather complete the new patient form at the time of your visit, please plan on arriving 15 minutes prior to your appointment time.

We will need to see a valid driver’s license or state photo identification (ID) card to verify your identity.
Relevant Medical Records
Please gather and bring any important medical records or laboratory test reports (x-rays, scans, etc.) from other physicians, hospitals, or health care providers that may be relevant to your care with us.

Please take a moment to write out and bring a list of questions and concerns you may have regarding pain management. This list will help you remember everything you wish to discuss during your visit.
Health Insurance Card
Please bring your health insurance card, so that we may bill your insurance correctly. Health plans we accept, but not limited to, include Aetna®, AmeriHealth®, Anthem BlueCross BlueShield®, Medicare, UnitedHealthcare®, and Tricare®. If you’re not covered by any of these plans, we accept most major health plans, including:
- Personal injury
- Workers’ compensation
- Motor vehicle accident
- Most major health plans accepted
